Role of Ribavirin in Membranoproliferative Glomerulonephritis Associated with Hepatitis C Virus Infection Refractory to Alpha-Interferon

Abstract: Chronic hepatitis C virus (HCV) has been associated with several extrahepatic diseases, such as membranoproliferative glomerulonephritis (MPGN). α-Interferon is currently the treatment of choice for this association. When this therapy fails clinicians face a difficult challenge due to the lack of useful information in these particularly difficult patients. We report the case of a severe nephrotic syndrome due to MPGN associated HCV infection, in which a triple association – interferon plus ribavirin and cyclop… Show more

“…Recently, hepatitis C virus infection has been increasingly implicated as a cause of type I MPGN, [19][20][21][22] and the association of type I MPGN with human immunodeficiency virus (HIV) or other viruses, such as Epstein-Barr virus, BK type polyoma virus, hepatitis G virus, or parvovirus B 19, have also been described in patients after renal transplantation. [23][24][25][26][27] Although serological tests for hepatitis C virus (HCV) antibody were negative in both cases, we did not test for HCV RNA and other viruses.…”

“…These findings were similar in several other small studies that also examined the effect of interferon-α in MPGN, but sample size was small and no trial had a control group. Isolated cases have also been reported of MPGN in association with HCV, where the MPGN was effectively treated only with the combination of interferon with ribavirin [27], but there have been no larger series or controlled studies. The aggregate of the data on interferon-based treatment for HCV-associated MPGN still lacks long-term outcomes on the benefits to renal function.…”
