Role of Radiation Therapy on the Use of Primary (“Neoadjuvant”) Systemic Treatment of Breast Cancer

Abstract: For LABC and inflammatory breast cancer the role of PST is well defined. For operable lesions, however, the value of preoperative radiotherapy still has to be established. This should be assessed within the framework of a clinical trial using standardized parameters for applying chemotherapy as well as radiation therapy.

“…When inoperability criteria prevails, RT should be used for tumour downstaging. Currently, neoadjuvant (NA) RT may be applicable for inoperable breast cancer to allow an effective and sometimes more conservative surgery 6 8. Besides, there is the benefit of eradicating occult micrometastatic disease.…”

“…Based on the results of several trials, postoperative breast cancer radiotherapy was considered mandatory and is a well-established management now, even in case of a noninvasive breast carcinoma [8,9,13,17,20,21]. In order to reduce local relapse and improve survival, adjuvant irradiation has been established [1,11,16].…”

“…Large randomized trials and statistical studies have established the benefit of PORT in breast cancer, with improved survival and reduced rate of local recurrence in appropriately selected patients [ 9 11 ]. Preoperative RT (PRRT), similar to neoadjuvant chemotherapy, may be applicable not only for patients with locally advanced breast cancer and inoperable advanced-stage tumors for BCS but also for those with operable tumors, aiming at breast conservation and higher complete remission rates [ 12 , 13 ]. However, neoadjuvant treatment may also stimulate CSC self-renewal through cytokine production and DNA repair mechanisms.…”
