Treatment of melasma remains challenging due to frequent relapses and the occurrence of post-inflammatory hyperpigmentation (PIH) following many interventions. Treatment goals are inhibition of melanin synthesis and transfer by sunscreens, sun avoidance and the use of bleaching agents on one hand and removal of already present pigment by peeling, lasers and light on the other hand. Triple combination creams (TCC) (also known as Kligman's formula utilizing hydroquinone and modifications of Kligman's formula) remain the mainstay of treatment. Newer bleaching agents derived from plants and synthetic compounds have been introduced and many of them are promising safe substitutes for hydroquinone.Peeling agents especially superficial and medium depth peels are effective treatment modalities for melasma. Newer peeling agents are less irritating and may thus lead to fewer side effects. Q-switched lasers, which target melanin, have led to PIH until the innovation of "laser toning" and fractional Qswitched ruby, which have better outcomes. Fractional non-ablative and ablative lasers are useful at lower fluencies. Pulsed dye laser, which targets the vascular component in melasma, is promising with less melasma rebound.Combinations of the above-mentioned modalities usually have a better treatment outcome than either alone. This review will focus on the more recently introduced therapies.

ÖzetYoğun ışık kaynakları 400-1200 nm freaknslarda kohorent olmayan ışık sistemleridir. Lazerlerle benzer kullanım ve yan etki profiline sahiptir. Epilasyon, telenjiektazi, pigmentasyon ve fotoyaşlanma tedavisinde başarı ile kullanılmaktadır. Deride yanma, bül, hipo ve hiperpigmentasyon ve ağrı en sık görülen yan etkileridir. Geniş başlıkları olması nedeniyle geniş alanlarda kullanılabilmesi, koyu renklilerde daha az yan etkiye neden olması avantajı olarak görülmektedir. Birçok farklı tipte cihaz olması, etkinlik ve güvenliğinin tam olarak değerlendirilmesini zorlaştırmaktadır. Sum maryIntense pulsed light (IPL) devices use incoherent high-intensity pulsed light of 400-1200 nm wavelength spectrum. Indication and adverse reactions of IPL systems are similar to lasers. IPL systems can be used for epilation, telengiectasia, pigmentation and photoaging therapy. The most side effects due to IPLs are burning and pain on the skin, bulla formation, hipo and hyper pigmentation. IPL devices have advantages for wide lesions with large spot size, and they can be used for darker color skin with less side effect. Gi riflLazerin sağladığı, belirlenen hedefe ulaşabilen ve başka dokulara zarar vermeyen enerji aktarımı tıbbın diğer alanlarında olduğu gibi dermatolojik uygulamalarda da kullanılmaktadır. Lazer tekniği aynı frekanslı eşgüdümlü ışın demetinin aynı anda aynı hedefe ulaştırılmasını sağlar. Lazer ışıktan üretilir ve ışık gibi fotonlarla yayılır. Işık için geçerli olan optik kurallara uyar. Tıpta kullanılan lazerler görünür ışık, mor veya kızılötesi dalga boylarında olabilir. Lazerin ışıktan üç önemli farkı vardır. Lazer tek dalga boyundadır, demetlenmiş bir ışın dalgasıdır yani dağılmış değildir ve kohorent dediğimiz şekilde tüm dalgalar aynı fazdadır. Lazer sistemlerinin dermatolojide kullanılmasından hemen sonra aynı teknoloji ile üretilen ve belli frekanslardan oluşan ışın demetlerinin de birçok alanda kullanılabileceği düşünülmüştür. Bu amaçla kohorent olmayan ışık demeti olan ve dünya genelinde kısaca IPL (intensive pulse light) ismi verilen yoğun ışık kaynağı diye isimlendirebileceğimiz cihazlar da geliştirilmiştir 1,2 . Yoğun ışık kaynağı cihazlarının dalga boyları teorik olarak 400-1200 nm arasında olabilir. İlk çıkan cihazların dalga boyu aralığı, görünür alanın bir kısmı ile yakın infraruj dalga boylarından oluşmaktadır (550-1200 nm). Bu dalga aralığında 1000 nm ve üzerindeki uzun dalga boylarının suya afinitesi yüksektir. Epidermisin %70 oranında su içerdiği düşünüldüğünde, epidermiste ablatif istenmeyen reaksiyon beklentisi doğabilir. Background: Tranexamic acid and vitamin C derivatives are important reagent used in cosmetic skin-whitening products. Recently, self-perceptions regarding improvement in facial skin status have been increasingly recognized as an important outcome of whitening cosmetic products intervention. Aims: The purpose of the study was to determine the receptiveness of objective and self-perceived facial skin status using whitening cosmetic products. Methods: A total of 40 volunteers were photographed using the Visia ® machine and completed questionnaires at pre-and post-operation (8 weeks after receiving whitening cosmetic products). Visia ® digital image analysis and questionnaires were used for assessment of facial skin status. The receptiveness of objective and self-perceived measures was assessed by paired t test and Effect Size (ES) .
