Role of neutrophil to lymphocyte and monocyte to lymphocyte ratios in the diagnosis of bacterial infection in patients with fever

Abstract: PurposeTo study the role of the neutrophil:lymphocyte ratio (NLR) and monocyte:lymphocyte ratio (MLR) in discriminating between different patient groups hospitalized for fever due to infection and those without infection.MethodsFor 299 patients admitted to hospital for fever with unknown cause, a number of characteristics including NLR and MLR were recorded. These characteristics were used in a multiple multinomial regression analysis to estimate the probability of a final diagnostic group of bacterial, viral,… Show more

“…This can predispose to infectious complications later during hospital stay, as shown in our study. As NLR is known to be useful for the diagnosis of bacterial infections in patients with fever [41] , we showed significant associations of CRP concentrations and a trend to independent associations with sepsis.…”

“…16 The association of neutrophilic leucocytosis with bacteraemia and lymphocytosis with viremia is well known. 13 In the current study, the NLR was found to be higher in patients with bacterial infection compared to those with viral infection. However, in a study conducted on HIV and hepatitis C infected individuals, NLR was associated with higher illness severity.…”

“…12 These authors also found that NLR was a more convenient marker for infection than CRP, with a high specificity (83.9%) but moderate sensitivity for diagnosing septicaemia in critically ill patients. 13 Lymphocytes are cells that mount an immune response against microorganisms and other foreign substances. In the event of sepsis, a low lymphocyte count will detrimentally affect the immune response and eventually lead to increased morbidity.…”

“…Her physical examination was unremarkable including inconspicuous lung sounds. Her complete blood count suggested a viral infection from relatively decreased neutrophil (48%; norm: 35-75%) to lymphocyte (40%; norm: 20-59%) ratio [8]. The chest X-ray (CXR) results on 8 January were compatible with pneumonia with mild thickening lung marking at both lower lungs possibly because of crowded lung rather than infiltration.…”
