Role of laparoscopy in vaginal malformation

Abstract: Laparoscopy is not applied systematically in the management of any malformation, but it can be useful when complementary exams fail to make an accurate diagnosis of the anomaly or for the management of the rare complication of adhesions and bands. When used to search for endometriosis, MRI detects ~50% of lesions, but laparoscopy is certainly still appropriate for that purpose. However, the optimal timing of the procedure still needs to be established.

“…Endometriosis could be identified early in adolescent years by using clinical findings such as primary dysmenorrhea [17,18]. Although, no endometriotic findings were identified in a case series including 22 adolescents with vaginal malformations [19], in another case series endometriosis was present in 5/18 examined adolescents with vaginal obstructing anomalies [20]. According to the Endometriosis Association, two-thirds of the patients with endometriosis describe characteristic pelvic symptoms before 20 years, which are usually underevaluated [21].…”

“…Although its diagnostic accuracy in the management of congenital anomalies is not yet fully elucidated, it seems to be an extremely useful tool, especially in cases of obstructive or nonobstructive vaginal and/or cervical aplasia or dysplasia (8,11,24,25,(70)(71)(72)(73)(74)(75)(76)(77). All these improvements, together with advances in the field of endoscopy (e.g., office hysteroscopy, hydrolaparoscopy, better imaging in laparoscopy), greatly increase our efficacy for an accurate, clear, and detailed estimation of the anatomy of the female genital system (78)(79)(80)(81).…”

“…La coelioscopie n'est pas indispensable si une IRM a été pratiquée précédemment. Elle permet toutefois une meilleure observation de brides ou d'adhésions péritonéales [8]. La prescription d'ocytocine est un complément utile pour la bonne vidange utérine et d'un éventuel hématosalpinx.…”
