Role of IL28B genotyping in patients with hepatitis C virus-associated mixed cryoglobulinemia and response to PEG-IFN and ribavirin treatment

Abstract: The role of interleukin (IL) 28B in the treatment of chronic hepatitis C (CHC) has recently been examined in many studies, while a possible relationship between IL28B and the presence of mixed cryoglobulinemia (MC) remains to be clarified. In this study, we analyzed the influence of IL28B rs8099917/rs12979860 on the presence of MC and the role in treatment with PEG-IFN. We retrospectively examined 541 patients affected by CHC who were treated with pegylated interferon (PEG-IFN) and ribavirin from 2003 to 2012.… Show more

“…[36] Another study showed that Patients with the TT/CC genotype for rs12979860/rs8099917 had a significant correlation with higher HCVRNA levels. [37] Our results did not confirm this paradoxical association, but it shows that as we expect a better response to treatment. Favorable polymorphisms of rs12979860 are associated with a lower base line viral load.…”

“…Boglione et al 20 found the combined assessment of both IL28B SNPs useful in MC; nevertheless, the Authors included a consistent group of Egyptian patients presenting with different distribution and predictive value of IL28B with respect to Caucasians.…”

“… 18 Nevertheless, a different distribution of IL28B genotypes has been reported by a recent study, even though this result may be dependent on the geographical origin of the enrolled patients. 20 …”

“…Patients with CC or TT genotypes present a higher baseline viral load than CT/TT or TG/GG genotypes, despite a greater chance of viral clearance or response to PEG-IFN. Regarding this finding, in our previous published study we reported that the IL28B genotypes were related both to baseline viremia and to the presence or absence of mixed cryoglobulinemia [5]. Patients with the TT/CC genotype for rs12979860/rs8099917 showed a significant correlation with higher HCV-RNA levels, the presence of mixed cryoglobulinemia and insulin resistance.…”
