Role of F-18 FDG PET/CT in assessing bone marrow involvement in pediatric Hodgkin’s lymphoma

Abstract: F-18 FDG PET/CT can predict BMB results with high accuracy. F-18 FDG PET/CT may be used at initial staging of pediatric Hodgkin's lymphoma as it uncovers unsuspected BMI and BMB may be omitted in patients with PET-positive BMI.

“…Some of the false negative results in this study may be attributed to use of older generation PET/CT equipment [45]. The prevalence of BM invasion in advanced-stage HL was 16-18 % according to PET/CT and 3-6 % according to BMB [46,47]; among patients with early-stage HL and no B symptoms, the prevalence of BM invasion was less than 1 %.…”

“…Bone marrow biopsy is used for clinical staging decisions but obtains information from a limited area, typically the iliac crest. 18 F-FDG PET/CT provides information about the entire bone marrow, beyond clinical biopsy areas (Table 2) (35)(36)(37). Several authors reported that 18 F-FDG PET/CT accurately detected bone marrow involvement outside the pelvis in patients with negative results from routine biopsies of the iliac crest (35)(36)(37)(38) and have suggested that 18 F-FDG PET/CT can supplant marrow biopsy for purposes of staging.…”

“…In addition, in lymphoma, diffusely increased 18 F-FDG bone marrow uptake could be owing to inflammation rather than bone marrow involvement, and the reporting physician should be cautious in interpreting the diffuse pattern of uptake in this patient group. 88 …”
