Robotic Resection of Pancreatic Neuroendocrine Tumor

Abstract: Computer-assisted surgical devices, or "surgical robots," have recently been approved for general surgical use. The device allows the surgeon remote control of multi-articulated instrument arms with a three-dimensional view. Here we report the first known case of pancreatic resection with use of a computer-assisted, or robotic, surgical device. A 46-year old woman presented with back pain and a complex cystic mass in the tail of the pancreas. The daVinci surgical robot was used to remove the lesion en bloc wit… Show more

“…The first robotic pancreatic resection was reported by Melvin et al in 2003. 5 Since then, only one article dealing directly with robotic-assisted pancreatic resection has been published in the English literature. 6 In this article, we report the detailed technique of a totally robotic subtotal pancreatectomy for intraductal neoplasm of the pancreas.…”

“…Robotic pancreatic resection is mentioned rarely in the English literature, with only four articles found dealing with this procedure. [5][6][7][8] Among them, there are two case reports, 5,6 two articles from the same group, 5,7 and only two dealing directly with this procedure and with a brief description of the technique. 5,6 There is a lack of technical description of this complex procedure.…”

“…Giulianotti et al (7) published the first robotic pancreatic resection in Europe in 2003; in the same year, Melvin et al (8) described the first series of robotic pancreatic resection for neuroendocrine tumor in the United States. Since that time, studies have demonstrated that robotic PD (RPD) can be performed safely with low conversion rates, decreased morbidity and mortality, and a shorter hospital length of stay compared with open PD (OPD) (9)(10)(11).…”

“…40 Case series have been published on robotic pancreatectomy, pancreaticoduodenectomy and transaxillary thyroidectomy though none involved a comparison to open surgery. 39,[41][42][43] In colorectal surgery a small comparative trial of robotic versus laparoscopic total mesorectal excision for rectal cancer demonstrated a possible benefit of robotic surgery with a shorter length of hospital stay and similarly low complication rate between the two arms. 44 However robotic total colectomy is likely to be more difficult than conventional laparoscopic colectomy due to the need to access multiple quadrants of the abdomen during this procedure requiring redocking the robot mid-case.…”
