Robotic resection of giant left ventricular myxoma causing outflow tract obstruction

Abstract: We report a 38-year-old female, who presented with progressive dyspnea and fatigue. Echocardiography revealed a giant and freely mobile left ventricular myxoma causing left ventricular outflow tract (LVOT) obstruction. The patient underwent totally endoscopic robotic excision of a giant left ventricular myxoma. The tumor was completely removed through the mitral valve orifice with a left atriotomy incision.

“…Robotic‐assisted cardiac surgery has become a commonly used minimally invasive approach for mitral valve disease and atrial septal defects over the last two decades 1‐11 . Totally endoscopic robotic atrial septal defect (ASD) repair remains a highly complex procedure, which requires experience with several nonroutine operative steps, such as airway management and set‐up.…”

“…Nevertheless, right‐sided unilateral pulmonary edema developed in several patients. With the goal of providing better outcomes, the studies and alternative techniques in minimally invasive cardiac surgery were reviewed 1‐26 . The literature on robotic cardiac surgery presented no recommendations about the use of SL endotracheal tubes without lung isolation.…”

Funding informationNone.Cardiac myxomas have presented with pulmonary hypertension and congestive heart failure, and have been resected using robotic techniques. [1][2][3] We present images of a left ventricular myxoma presenting with mitral regurgitation and excised using an endoscopic approach. A 37-year-old male presented with palpitations and a grade 3/6 apical systolic murmur.
