Robotic parathyroidectomy

Abstract: Robotic parathyroidectomy has recently been described. Although the procedure eliminates the neck scar, it is technically more demanding than the conventional approaches. This report is a review of the patients' selection criteria, technique, and outcomes.

“…18 At most institutions, this procedure was performed with a robotic camera and 2-3 robotic arms through a single 50-to 60-mm longitudinal incision in the axilla. [18][19][20][21][22][23][24] CO 2 insufflation is not used; instead, a bladed retractor is attached to secure a visual field for surgery. Although one article described two cases of robot-assisted subtotal parathyroidectomy through a unilateral axilla, 25 our systematic literature search identified no other articles that reported bilateral exploration with the axillary approach.…”

“…Based on the articles in this review, the applications of RAP for PHPT are as follows: cases of a single parathyroid adenoma, 1,10,13,22-24,28,48,49 sporadic PHPT, 22,24 those with localization of a parathyroid adenoma identified by preoperative studies, 1,10,13,16,[22][23][24]28,31,45,48,49,52,53,62,63 and patients with a history of hypertrophic scarring. 46,52 Most of the articles dealing with PHPT described a single parathyroid adenoma as the indication for RAP, and this is correct for surgeons who are inexperienced regarding remote-access surgery.…”

“…The inherent limitations of cervical and extracervical endoscopic surgical approaches have lead to the emergence of robotic parathyroidectomy as an alternative option (27). The causes of this limitation are mainly related to the technical difficulties encountered with conventional endoscopic surgery, for which the surgeon uses instruments without articulated ends, which thus provide only four degrees of freedom.…”

“…Application of robotic technology for parathyroid removal has been less extensively studied and its use has not been widely adopted. Previous studies have documented the safety of the procedure with the advantages of greater image magnification, gasless technique, and a hidden axillary incision (1)(2)(3)(4). Herein we present our technique of robotic transaxillary parathyroidectomy at an academic institution.…”
