Цель. Выявить наиболее значимые факторы кардиоваскулярного риска у больных с острым коронарным синдромом (ОкС) и почечной дисфункцией (Пд), перенесших чрескожное коронарное вмешательство (ЧкВ). Материал и методы. В исследование включено 206 пациентов. критерием Пд явилось снижение скорости клубочковой фильтрации (СкФ) <60 мл/ мин/1,73 м 2 , рассчитанного по формуле CKd-ePI (2011) на основании концен-трации креатинина крови при поступлении стационар. Госпитальная леталь-ность составила 13,1% (n=27). Годовая летальность от любых причин соста-вила 15,5% (n=32), 3-х летняя -21,8% (n=45). Результаты. Однофакторный анализ установил, что наиболее значимыми факторами госпитальной и отдаленной летальности у больных с ОкС и Пд, перенесших ЧкВ, явились застойная сердечная недостаточность, инсульт и инфаркт миокарда в анамнезе, острая сердечная недостаточность (ОСН), гипергликемия, выраженная систолическая дисфункция миокарда, тахикардия и гипотония при поступлении в стационар, многососудистое поражение коронарных артерий, а также острое повреждение почек (ОПП), развившееся в течение госпитализации (критерии критериям rIFLe и AKIN). Пошаговый отбор в регрессии кокса показал, что факторами, наличие которых связано с повышением частоты госпитальной летальности, явились инсульт в анамнезе, ОСН на момент поступления, а также развитие ОПП в течение госпитализации. С-статистика полученной модели составила 0,82, что значи-тельно больше, чем у шкалы grACe 2,0-0,74. При проведении анализа долгосрочной выживаемости кокса выявлено, что наличие в анамнезе инсульта и инфаркта миокарда, а также ОСН при посту-плении в стационар и ОПП независимо от других факторов связаны с повыше-нием частоты смертельных исходов в течение одного и трех лет наблюдения. При этом площадь под roC-кривыми собственных моделей риска составила 0,84 и 0,76, соответственно, для года и трех лет. У шкалы grACe 2,0 данный показатель достоверно ниже -0,78 и 0,69, соответственно. Заключение. Приведенные результаты свидетельствуют о том, что у больных ОкС и исходной Пд необходимо учитывать дополнительные факторы при стратификации риска после ЧкВ, а разработанные новые модели оказались достоверно лучше шкалы grACe. Acute coronary syndrome (ACS) is associated with both short-and long-term unfavorable prognosis. Therefore, medical societies developed risk scores for predicting mortality and assessing decision-making regarding early aggressive treatment in patients presenting an ACS. The Thrombolysis In Myocardial Infarction and the Global Registry of Acute Coronary Events risk scores are the most extensively investigated scores for ACS. Clinical judgment is also important. Significant differences in aggressive treatment of ACS still exist with respect to gender, age, and ethnicity. The reasons for these discrepancies need to be further elucidated in future studies. Therefore, generalizability of stratifications and risk scores in certain populations should be performed with caution.Keywords acute coronary syndrome, mortality risk scores, Thrombolysis In Myocardial Infarction (TIMI) risk score, Global Registry of Acute Coronary Events (GRACE) risk score, primary percutaneous coronary intervention Acute coronary syndrome (ACS) is associated with both shortand long-term unfavorable prognosis. Therefore, accurate prediction of mortality risk in patients presenting with an ACS is very important for decision-making regarding early aggressive treatment (mainly primary percutaneous coronary intervention [pPCI]). Significant differences in aggressive treatment of ACS exist with respect to gender, age, and ethnicity. [1][2][3] In this context, studies evaluating pPCI in women with ACS suggested that women undergo pPCI less often than men.4-7 However, there are some conflicting results reporting similar or even higher rates of percutaneous coronary intervention (PCI) in women with ACS as those derived from the Coronary Angiography and PCI Registry of the German Society of Cardiology. 8It should be noted that women with stroke are also less treated with intravenous thrombolysis than men 9 and that gender differences have been reported for both traditional and emerging cardiovascular risk factors [10][11][12] ; risk factors are less frequently and less aggressively treated in women than in men. 13With regard to age, elderly patients with ACS may have atypical symptoms, severe comorbidities affecting drug metabolism and their functional status, and an increased risk for cardiovascular mortality.14 These age-related characteristics may negatively influence their treatment strategy and outcomes. Older patients with ACS can benefit from (and therefore should not be deprived of) early coronary revascularization. 15,16 Interestingly, a 35-year-old man, cigarette smoker without electrocardiogram abnormalities but with an elevated cardiac troponin (cTN) concentration may have a Global Registry of Acute Coronary Events (GRACE) score *80, whereas an elderly person with elevated cTn and nonspecific fixed electrocardiographic changes will have a GRACE score >140.
