Risk of Stroke in Women Exposed to Low-Dose Oral Contraceptives

Abstract: These results cast doubt on a true association between low-dose OCPs and stroke because of the low absolute magnitude of the ORs, the severe methodological limitations, and the ORs of less than 1.0 in the cohort studies. The association is tenuous at best and perhaps nonexistent.

“…• Contraception et traitement hormonal substitutif Le rôle des hormones féminines dans la survenue de la pathologie cérébrovasculaire a été également étudié. Ainsi, la contraception orale est associée à un risque accru d'IC avec un risque relatif compris entre 2 et 4 [24]. L'association existe quel que soit le type de pilule et son dosage en oestrogènes, même si elle est plus faible avec les pilules microdosées (< 50 μg d'oestrogènes).…”

“…In the research by Chan et al, the risk of stroke due to consumption of OCPs (0.51 -1.76) was related to thrombotic but not hemorrhagic stroke. Besides, the incidence of stroke was doubtful in low doses of OCPs (11). In another study, the risk of death within 28 days after the incidence of myocardial infarction was 2.88 folds (1.22 -6.77) higher among the consumers of OCPs compared to the non-consumers.…”

“…In a systematic meta-analysis, Chan et al investigated 20 studies and showed that the risk of stroke as the result of OCPs was only related to thrombotic but not hemorrhagic stroke (11). Furthermore, another study indicated that consumption of OCPs increased the risk of stroke in migraine sufferers, irrespective of other risk factors (12).…”

“…Cerebrovascular accident (CVA) is another very rare blood disease among women at the reproductive age 43 . However, there is a higher incidence of cases among users of COC compared to non-users 44,45 . When Heinemann 46 examined epidemiological articles published, it was possible to conclude that high doses of EE (≥ 50 mcg) were associated with increased risk of stroke compared to formulations with 50 mcg of EE (OR:5.3; 95% CI: 2.6-11 versus OR: 1.53; 95% CI: 0.71-3.31) 47 .…”
