estimates for the comparison between exposed and unexposed. 4 Furthermore, it is important to note that we used incident outcomes to reduce bias from pre-existing mental health problems, meaning that individuals with a history of diagnoses for a given outcome were excluded from the model for that specific outcome. In other words, the rate of any neuropsychiatric outcome was for individuals who did not have previous neuropsychiatric diagnoses. Incidence rates by 3-month intervals showed comparable associations between the cohorts than what was seen for the 12-month incidence.The recommended duration of isotretinoin treatment is up to 20 weeks in the USA and up to 24 weeks in the UK. For many, one course of treatment with a low dose and short duration is effective. Many of the physical side-effects of isotretinoin occur shortly after treatment initiation. It can be assumed that the same timing applies to potential psychiatric outcomes. It is well established that the highest risk of side-effects is associated with a new treatment episode and that sustained drug use is an indication of tolerability. The one-year follow-up with new treatment episode was thus justified based on clinical practice and previous evidence on isotretinoin.A recent study in Taiwan showed that higher dose or longer duration of isotretinoin treatment were not associated with the risk of neuropsychiatric outcomes. 5 Our dataset did not include information on dose, and we welcome future research focusing on dose and duration of treatment in different populations.
