Risk of metastasis in adenocarcinoma of the esophagus: a multicenter retrospective study in a Japanese population

Abstract: Mucosal and submucosal cancers (1-500 µm invasion) without risk factors have a low incidence of metastasis and may thus be good candidates for endoscopic resection.

“…A recent study in Japan [81] validated these criteria, showing no metastases (0/186 lesions) in patients with mucosal cancer without lymphovascular involvement and a poorly differentiated component, or in patients with SM cancer (≤ 500 μm) without lymphovascular involvement, a poorly differentiated component, and ≤ 30 mm (0/32 lesions).…”

“…CR was defined upon satisfying the following criteria: negative horizontal margin, negative vertical margin, negative vascular invasion, and depth of submucosal invasion (SM) ≤500 μm. Invasion distance of the criteria was defined according to a recent multi-DOI: 10.1159/000486197 center retrospective study in a Japanese population about SM invasion depth and lymph node metastasis rate of esophageal adenocarcinoma [17]. As secondary endpoints, we evaluated resection duration, complications, en bloc resection rate, negative horizontal margin rate, R0 resection rate, noncurative factors, and prognosis after the first ESD including overall survival and disease-free survival rates.…”

“…In Japan, the main therapeutic approaches for EGJ adenocarcinomas are endoscopic resection and surgery. According to a recent report, endoscopic resection was carried out in over half of cases with esophageal adenocarcinomas, including EGJ adenocarcinomas, which were confined to the submucosa . Endoscopic screening and precise diagnosis may contribute to detection at an early stage.…”
