Risk of Infective Endocarditis in Patients with End Stage Renal Disease

Abstract: Patients receiving RRT have a high incidence of endocarditis, in particular during hemodialysis treatment using central venous catheters. The first 6 months in RRT, aortic valve disease, and previous endocarditis are significant risk factors for developing endocarditis.

“…In patients with End Stage Renal Disease (ESRD) requiring hemodialysis, the incidence of endocarditis of bioprosthetic or mechanical valves was 7.8% and 5.4% respectively, which is ten to fifteen times higher than the incidence of endocarditis in patients with chronic kidney disease (CKD), which was about 0.41% [21,22]. Endocarditis can be a major but rare complication of cardiac device implantation; for example, in patients with pacemakers and defibrillators, the rate of endocarditis is 0.38% [23].…”

“…In addition to this, excessive alcohol consumption may be an independent risk factor for E. coli IE. Patients undergoing renal replacement therapy also have a high incidence of endocarditis [ 35 ]; therefore, it should be noted that these comorbidities (diabetes, malignancy, excessive alcohol consumption, and hemodialysis) may be risk factors for E. coli IE. Preceding urinary tract infection was reported in 52% (17/33) of the patients [ 12 15 , 20 , 21 , 25 , 28 , 30 32 ].…”

“…6,7 In the present case, RCC was present but CRP level was not increased, so we attempted to identify alternative potential causes of the fever. IE is an important cause of prolonged fever 8 and it is more common in hemodialysis patients, 9 but its classical symptoms may not be apparent and it may thus be easily overlooked. 1 The current patient was at high risk of IE after pacemaker implantation, 10 but the possibility of IE was considered low based on her echocardiogram and blood culture results.…”
