Risk of hospitalization and death from COVID-19 infection in patients with chronic plaque psoriasis receiving a biologic treatment and renal transplant recipients in maintenance immunosuppressive treatment

Abstract: IRB approval status: Because of the retrospective nature of the study, only a notification to the ethical committee of the principal investigator center (Fondazione IRCCS Ca' Granda Ospedale Maggiore Policlinico, Milan, Italy) was requested. All patients gave written informed consent for publishing the clinical images.

“…[1][2][3] Recently, few papers describe chilblain-like lesions (CLL) as a possible skin clue of COVID-19 among young patients and children, mostly in the absence of systemic symptoms and with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) swab negativity. [4][5][6][7][8] This observation is consistent with the atypical outbreak of CLL reported in high endemic COVID-19 Italian regions among paediatric and dermatological networks. 6 We hereby report a case of an adolescent boy with a reverse transcriptase (RT)-PCR-confirmed COVID-19 who developed long-lasting CLL.…”

“…Our case reflects the prototype reported in the literature: an adolescent in good health with occasional history of systemic symptoms preceding cutaneous lesions. [4][5][6][7][8] Interestingly, we observed in our patient long-lasting skin lesions several weeks after the first showings of the symptoms, concurrently with the positivity of the nasopharyngeal swab.…”

“…This happened despite recommendations of the Italian Society of Dermatology -SIDeMaST (10). Indeed, no increased risk of hospitalization or death from COVID-19 was recently reported in psoriatic patients treated with biological therapies (21)(22)(23)(24). On the other hand, worsening of the disease has been also experienced by patients who continued the treatment and this was due mainly to the psychological distress deriving by the lockdown (25) as pointed out by the correlation found between HADS, BRS and PSS alterations and the worsening of psoriasis, confirming the wellknown relationship between psychological distress and the cutaneous immune response (26,27).…”

“…8,9 To date, there is neither an agreement nor a study sustaining the impact of continuing or stopping biologics in psoriatic patients during the COVID-19 pandemic. [10][11][12][13][14][15][16] Furthermore, data on the epidemiology of COVID-19 disease in patients treated with biological drugs are very scarce. 8 A C…”
