Risk of fracture in transfusion-naïve thalassemia population: A nationwide population-based retrospective cohort study

Abstract: In thalassemia major or transfusion-dependent thalassemia patients, osteoporosis-related bone complications such as fracture events are common. However, no studies have investigated the risk of fracture in transfusion-naïve thalassemia population. Therefore, we conducted a longitudinal nationwide cohort study to determine whether this population has an increased risk of fracture. This nationwide, population-based cohort study analyzed data from 1998 to 2010 obtained from the Taiwanese National Health Insurance… Show more

“…We conducted this retrospective cohort study using the Longitudinal Health Insurance Database 2000 (LHID 2000) from the Taiwanese National Health Insurance (NHI) program that enrolls more than 23 million individuals, representing approximately 99.9% of the population of Taiwan [ 13 , 14 ]. The LHID 2000 and NHI program details have been described in previous studies [ 15 , 16 ]. The Research Ethics Committee of China Medical University and Hospital in Taiwan approved the study (CMUH104-REC2-115-CR3).…”

“…Infection is a major issue for patients with the severe form of thalassemia, and the proposed predisposing factors are anemia, iron overload, splenectomy, transfusion-associated viral infections, and a range of immune abnormalities (2). For thalassemia patients without a history of blood transfusion, although there may be no symptoms of anemia or it may be mild, such as thalassemia minor patients, they also have a higher risk of coronary artery disease, erectile dysfunction, and fractures (21)(22)(23). Their hematopoiesis may be less effective, and the abnormal erythrocyte cell membrane structure leads to peripheral hemolysis and subsequent iron overload, which are risk factors for susceptibility to infection (11,12).…”

“…The Taiwan NHI program has covered 99% of the Taiwan population (including 23.74 million) since 1995 and is thus a thorough and representative sample of the population [ 16 ]. Details of the NHI program and LHID2000 have been well documented [ 17 , 18 ].…”
