Risk for Unplanned Hospital Readmission of Patients With Cancer: Results of a Retrospective Medical Record Review

Abstract: The factors identified provide a basis for assessment, planning, interventions, and follow-up of patients to help reduce the risk of readmission and, thus, poor outcomes.
