Risk factors of systemic inflammation response syndrome after endoscopic combined intrarenal surgery in the modified Valdivia position

Abstract: The number of involved calyces larger than four, stone surface area >500 mm(2) and a history of febrile urinary tract infection independently predicted the development of systemic inflammation response syndrome after endoscopic combined intrarenal surgery. This is the first study to identify the independent predictors of systemic inflammation response syndrome after endoscopic combined intrarenal surgery in the modified Valdivia position.

“…Tabei et al speculated that this discrepancy might be explained by the lower intrarenal pressures obtained by simultaneous ureteroscopy (and the presence of the ureteral access sheath they insert). 2 We totally agree, also considering the significantly higher septic shock rates of the miniaturized percutaneous approaches (small tracts in the absence of an adequate irrigation out flow). 5 Therefore, the combination of PCNL with retrograde ureteroscopy in ECIRS does not imply the sum of the two infectious risks, with the retrograde one being rather low (1-10%) but possibly life-threatening if not treated in a timely manner or if underestimated.…”

“…In a previous study of 370 ECIRS cases, a history of febrile UTI increased the risk of sepsis. 25 Therefore, the prevention of postoperative febrile UTI is important to achieve better outcomes following ECIRS.…”

“…However, severe postoperative infections can occur with ECIRS, such as those with PCNL and URS. 6 In the guidelines published by the Japanese Urological Association, 7 BL inhibitor-containing penicillin, firstand second-generation cephalosporins, and aminoglycosides are recommended for URS and PCNL. There is still no mention of ECIRS in the European Association of Urology, the American Urological Association, and the Japanese Urological Association guidelines.…”
