Risk factors for renal dysfunction in the postoperative course of liver transplant

Abstract: Renal dysfunction (RD) is a frequent complication after orthotopic liver transplantation (OLT), and it has an unfavorable effect on the prognosis of OLT patients. The purpose of our study was to identify possible risk factors for RD and its impact on survival. The possible relations of pre-, peri-, and postoperative variables to early-onset renal dysfunction (ED) (within the 1st 3 months), lateonset renal dysfunction (LD) (between 3 and 6 months), and chronic renal dysfunction (CRD) (beyond 6 months) was analy… Show more

“…Intraoperative blood transfusion is known to affect postoperative renal outcomes in OLT patients [1820]. A biologically plausible explanation for this association is that kidney hypoperfusion secondary to intraoperative bleeding represents an additional renal insult in patients with pre-OLT AKI.…”

“…The incidence of post-operative AKI in the setting of LT ranges between 12% and 80%, depending on the definition adopted[17,18], with a 30-d mortality of up to 50% in case of renal dysfunction at transplantation and as high as 90% if renal replacement therapy (RRT) is required[4,19]. …”

“…Cold ischaemia time (CIT) and graft steatosis have been reported to be associated with PRS in several studies[6,13,17,21,22]. The increased CIT is likely to increase the IRI which is associated with release of increased amounts of pro-inflammatory mediators, generation of free radicals, neutrophil sequestration and activation[50].…”

“…It affects between 25% and 60% of recipients[2-4], the variation being largely related to the definition of AKI utilised[5,6]. The incidence of AKI following OLT is much higher than with other non-cardiac major surgery, in patients with previously normal renal function[7].…”
