Risk factors for primary biliary cirrhosis in a cohort of patients from the United States

Abstract: Primary biliary cirrhosis (PBC) is an uncommon and enigmatic autoimmune disease, characterized by female predominance, autoantibodies to mitochondrial antigens (AMA), destruction of small bile ducts, and ultimately cirrhosis and liver failure. 1,2 Although genetic factors appear to be important in determining susceptibility to autoimmune diseases such as PBC, studies of a variety of autoimmune diseases in monozygotic twins have revealed concordance rates of only 10% to 50%. 1,2 Thus, environmental factors may … Show more

“…Cigarette smoking, another source of benzene and other volatile organic compounds, and chronic bronchitis are associated with PBC. 12,36 Interestingly benzene is known to induce both hepatic oxidative stress 37 and immune dysfunction. 38,39 Additionally, women have a higher pulmonary air/blood partition coefficient 40 for benzene and female gender enhances hepatic 37 sensitivity to benzene.…”

“…[11][12][13][14] Immune-related gene polymorphisms are increased in frequency in PBC. 15 Also, the concordance rate of PBC among monozygotic twins (60%) is significantly higher than among dizygotic twins (0%).…”

“…The mechanisms for such increased sensitivity are currently unknown but might reflect or be secondary to the higher frequency of recurrent gram-negative bacterial infections reported among patients with PBC. 26 A series of negative regulators of the TLR pathways of activation and signaling should be noted. 27 Defects may be present in 1 or more of these pathways in PBC patients that could be the basis of enhanced cytokine synthesis.…”

“…11 We report herein the results of the largest case-control study to date of environmental, familial, and medical factors associated with PBC. The study is based on a structured, standardized telephone interview, administered by trained personnel to 1032 patients with PBC from the United States and 1041 unrelated controls selected by random-digit-dialing (RDD) and matched for sex, age, race, and geographical location.…”
