Coronavirus disease 2019 (COVID-19) continues to spread rapidly all over the world challenging nearly all governments. The exact nature of COVID-19’s spread and risk factors for such a rapid spread are still imprecise as available data depend on confirmed cases only. This may result in an asymmetrically distributed burden among countries. There is an urgent need for developing a new technique or model to identify and analyze risk factors affecting such a spread. Fuzzy logic appears to be suitable for dealing with multi-risk groups with undefined data. The main purpose of this research was to develop a risk analysis model for COVID-19’s spread evaluation. Other objectives included identifying such risk factors aiming to find out reasons for such a fast spread. Nine risk groups were identified and 46 risk factors were categorized under these groups. The methodology in this study depended on identifying each risk factor by its probability of occurrence and its impact on viruses spreading. Many logical rules were used to support the proposed risk analysis model and represented the relation between probabilities and impacts as well as to connect other risk factors. The model was verified and applied in Saudi Arabia with further probable use in similar conditions. Based on the model results, it was found that (daily activities) and (home isolation) are considered groups with highest risk. On the other hand, many risk factors were categorized with high severity such as (poor social distance), (crowdedness) and (poor personal hygiene practices). It was demonstrated that the impact of COVID-19’s spread was found with a positive correlation with the risk factors’ impact, while there was no association between probability of occurrence and impact of the risk factors on COVID-19’s spread. Saudi Arabia’s quick actions have greatly reduced the impact of the risks affecting COVID-19’s spread. Finally, the new model can be applied easily in most countries to help decision makers in evaluating and controlling COVID-19’s spread.

We analyzed the clinical characteristics of outpatients with influenza-B-associated pneumonia during the 2021–2022 influenza season and analyzed the molecular epidemiology and evolution of influenza B virus. The presence of influenza B virus was confirmed by reverse transcription polymerase chain reaction (RT-PCR). Electronic medical records were used to collect and analyze data of outpatients. The HA and NA genes were phylogenetically analyzed using ClustalW 2.10 and MEGA 11.0. Out of 1569 outpatients who tested positive for influenza B virus, 11.7% (184/1569) developed pneumonia, and of these, 19.0% (35/184) had underlying diseases. Fever, cough, and sore throat were the most common symptoms. Among the complications, acute respiratory distress syndrome (ARDS), acute kidney injury (AKI), and shock accounted for 2.7% (5/184), 4.9% (9/184), and 1.6% (3/184), respectively. Of the outpatients, 2.7% (5/184) were admitted to the hospital, and 0.5% (1/184) of them died. All of the strains from Beijing were identified as belonging to the B/Victoria lineage. The HA and NA gene sequences of 41 influenza B viruses showed high similarity to each other, and all of them belonged to clade 1A.3. Compared with the vaccine strain B/Washington/02/2019, all of the isolates contained N150K, G181E, and S194D mutations. S194D, E195K, and K200R mutations were detected in the 190 helix of the receptor binding region of HA. Co-mutations of H122Q, A127T, P144L, N150K, G181E, S194D, and K200R in HA and D53N, N59S, and G233E in NA were detected in 78.0% (32/41) of the isolates, and 56.3% (18/32) of these were from outpatients with influenza-B-associated pneumonia. Influenza outpatients with underlying diseases were more likely to develop pneumonia. No significant differences were observed in clinical symptoms or laboratory results between outpatients with and without pneumonia, so testing for influenza virus seems to be a good choice. The observed amino acid variations suggest that current vaccines might not provide effective protection.

Viral pneumonia has been a serious threat to global health, especially now we have dramatic challenges such as the COVID-19 pandemic. Approximately six million cases of community-acquired pneumonia occur every year, and over 20% of which need hospital admission. Influenza virus, respiratory virus, and coronavirus are the noteworthy causative agents to be investigated based on recent clinical research. Currently, anaphylactic reaction and inflammation induced by antiviral immunity can be incriminated as causative factors for clinicopathological symptoms of viral pneumonia. In this article, we illustrate the structure and related infection mechanisms of these viruses and the current status of antiviral therapies. Owing to a set of antiviral regiments with unsatisfactory clinical effects resulting from side effects, genetic mutation, and growing incidence of resistance, much attention has been paid on medicinal plants as a natural source of antiviral agents. Previous research mainly referred to herbal medicines and plant extracts with curative effects on viral infection models of influenza virus, respiratory virus, and coronavirus. This review summarizes the results of antiviral activities of various medicinal plants and their isolated substances, exclusively focusing on natural products for the treatment of the three types of pathogens that elicit pneumonia. Furthermore, we have introduced several useful screening tools to develop antiviral lead compounds.
