Risk factors for hepatocellular carcinoma at baseline and 1 year after initiation of nucleos(t)ide analog therapy for chronic hepatitis B

Abstract: Nucleos(t)ide analogs (NAs) cannot completely suppress the risk of hepatocellular carcinoma (HCC) in patients with chronic hepatitis B (CHB). This study aimed to identify the risk factors for HCC development in naïve CHB patients treated with current NA. Patients receiving NA (n = 905) were recruited retrospectively from the 17 hospitals of the Japanese Red Cross Liver Study Group. All treatment‐naïve patients had been receiving current NA continuously for more than 1 year until the end of the follow‐up. We an… Show more

“…The median (IQR) HBV DNA levels were undetectable (undetectable, 1.0 log IU/mL). Aspartate aminotransferase (AST) and alanine aminotransferase (ALT) levels were in the normal range (AST, 23 [20][21][22][23][24][25][26][27] IU/L; ALT, 20 [15][16][17][18][19][20][21] IU/L). Previous ETV treatment was for 72 (58-108) months.…”

“…HBV DNA was detectable in about 20% of patients during treatment with NAs 13 . Host characteristics such as age and sex were found to be associated with HCC development 18–20 . Regarding the viral factor for HCC development, LLV is a known risk factor for HCC development, 10 and avoiding LLV reduces the risk of HCC development in patients with chronic hepatitis B 10,11,21,22 .…”

“…Liver fibrosis is the most important factor associated with HCC development and prognosis in patients with chronic hepatitis B and other chronic hepatitis. 20 , 21 , 22 Therefore, accurate evaluation of liver fibrosis is crucial in clinical practice. Several noninvasive modalities for liver fibrosis assessment have been developed.…”
