Risk Factors and Prophylactic Tenosynovectomy for Extensor Tendon Ruptures in the Rheumatoid Hand

Abstract: The cases histories of 51 rheumatoid arthritis patients (58 hands) were examined retrospectively with respect to the incidence of tendon rupture. Factors that were associated with tendon rupture such as X-ray changes in the wrist joint and clinical findings of the hand preceding the tendon rupture were statistically analysed. In a separate study, prophylactic tenosynovectomy and wrist synovectomy were performed on 42 joints in 35 patients who had two or more risk factors. This group of patients was then analys… Show more

“…The importance of ongoing tenosynovial inflammation in promoting tendon rupture has been confirmed by observations that prophylactic dorsal tenosynovectomy can prevent this complication in many patients (5). However, the decision to perform surgery is usually made in established cases of RA once clinical or radiologic features of joint damage have developed (6).…”

Therefore, along with severe extensor tenosynovitis [ 14 ] and scallop sign (not signifi cant in this study) in the radiograph [ 15 ], early recognition of wrist deformity and palmar dislocation of ECU tendon may "potentially" decrease the rate of unsuspected extensor tendon ruptures in RA (Table 13.1 ).…”

“…The aim of treatment is to relieve symptoms and prevent tendon rupture. Apart from the duration of symptoms, Ryu identifi ed two radiological prognostic factors for the risk of tendon rupture [ 11 ]: the scallop sign as described by Freiberg and dorsal dislocation of the distal ulna ( Fig. 11.7 ).…”
