Risk factors and prognosis in very low birth weight infants treated for hypotension during the first postnatal week from the Korean Neonatal Network

Abstract: Hypotension in the early stages of life appears in 20% of very low birth weight (VLBW) infants. The gestational age and birth weight are the risk factors highly related to the postnatal hypotension. Other risk factors slightly differ between different studies. So, we evaluated the risk factors and prognosis that are associated with infants treated with hypotension in the early stages of life, after excluding the influences of gestational age and small for gestational age (SGA). VLBW infants registered in the K… Show more

“…Postnatal factors include mechanical ventilation, postnatal steroids, patent ductus arteriosus, supplemental oxygen, and sepsis ( 24 ). In addition to well-known risk factors, in this study, low BP ( 25 ) and PHT ( 26 ), which required treatment within one week, were important as early factors for BPD. Previous studies ( 25 , 26 ) have shown that the above variables play a role in the risk factors of BPD, but their importance is higher than expected, so attention should be paid to premature care and additional studies are needed in the future.…”

“…No other antenatal/perinatal factor was significantly associated with AH and treatment with anti-hypotensive medications in very preterm infants after adjustment for confounders [6,[29][30][31]. Several studies confirmed the positive association of AP in preterm infants with the GA and/or BW, which were the strongest factors influencing neonatal AP [2,29,[32][33][34]. In line with these reports, we found an inverse correlation between the GA and the prevalence of AH as well as treatment with vasoactive medications, regardless of the age at treatment initiation.…”

“…It has even been characterized as idiopathic or permissive hypotension. Previous authors, however, demonstrated that certain prenatal/perinatal factors may be associated with the development AH in VLGAI [2]. In addition, AH has been linked to severe acute complications of prematurity, such as peri-intraventricular hemorrhage (IVH), respiratory distress syndrome (RDS), sepsis, necrotizing enterocolitis (NEC), and increased mortality, as well as severe long-term morbidities and poor neurodevelopmental outcomes in survivors [3][4][5][6][7].…”
