Background: Chorionic villus sampling (CVS) is used routinely as a first trimester diagnostic procedure for fetal karyotyping in at-risk pregnancies. The success of the procedure is dependent on the experience of the operator performing it. The objective of this study was to determine the relationship between an operator-controlled clinical and laboratory setting and the safety and reliability of CVS service. Patients and Methods: One thousand patients had a CVS procedure for a variety of indications, such as advanced maternal age, previous child with chromosome abnormality, etc. Both transcervical and transabdominal procedures were performed, according to placental location and uterine environment. For cytogenetic diagnosis, direct and short-term cultures were set up according to standard laboratory protocol. Result: Cytogenetic results were obtained in 99.6% of studies with 94.5% normal (46, XX or 46, XY), with the remaining having a variety of numerical and structural chromosomal abnormalities. Maternal cell contamination was found in 2% of the first 262 cases, while the overall rate observed in the 1000 samples was 0.5%. Level II mosaicism was observed in 0.8% and level III mosaicism observed in 0.9% of cases, respectively. The overall rate of pregnancy loss of chromosomally normal pregnancies within 28 weeks of gestation was 2.8%. No limb reduction defects were seen in any infant post-CVS. Conclusion: Our record demonstrates that experienced operators can deliver a safe and reliable CVS service.

ENGLISH SUMMARYAmniocentesis or chorionic villus sampling are performed in about 2% of all pregnancies in Norway which is far less than in the other Nordic countries. Most of the amniocenteses are performed due to advanced maternal age. In Norway this is defined as maternal age ≥ 38 years at term. Couples with known chromosomal aberrations or genetic diseases in their families, i.e. women at a high risk of having an affected fetus, are offered chorionic villus sampling. Earlier studies on complications and pregnancy outcome following amniocentesis or chorionic villus sampling have been performed in other countries mainly on women at a lower risk (≥ 35 years) than for the women having such tests in Norway. We do not have data on pregnancy outcome and possible complications following amniocentesis and chorionic villus sampling in a Norwegian population.

Fragestellung: Das Ziel der Studie war es, durch eine Kosten-Nutzen-Rechnung zu klären, ob ein Serum-Screening nach fetaler Trisomie 21 (Triple-Test) in Österreich empfehlenswert ware. Methoden: Die Triple-Test-Ergebnisse internationaler Studien der letzten Jahre wurden mit den Daten des Steirischen Fehlbildungsregisters der Jahre 1985–1992 kombiniert. Vier Faktoren wurden zu einer Kosten-Nutzen-Analyse herangezogen: Die Gesamtkosten der pränatalen Diagnostik, die Kosten pro diagnostizierten Fetus mit Trisomie 21 die Zahl der entdeckten Feten mit Trisomie 21 und die Zahl der eingriffsbedingten Fehlgeburten. Ergebnisse: Wenn man der Kostenfrage Priorität einräumt, so sollte der Triple-Test Schwangeren im Alter von > 35 Jahren angeboten werden. Wird eine maximale Entdeckungszahl von Feten mit Trisomie 21 als Priorität erachtet, sollte der Triple-Test Schwangeren aller Alters-gruppen angeboten werden. Schlussfolgerung: Die derzeit gängige Praxis des Angebotes einer fetalen Karyotypisierung an Schwangere im Alter von > 35 Jahren ist den Szenarien mit Triple-Test unterlegen.
