Ring-enhancing lesions in neonatal meningitis: an analysis of neuroradiology pitfalls through exemplificative cases and a review of the literature

Abstract: Very often the clinical course of neonatal meningitis (NM) is characterized by sudden worsening, at times associated with cerebrovascular complications and strokes or the formation of cerebral abscesses. The immediate recognition of these pathological patterns is pivotal in providing clinicians with useful information to differentiate between those different pathological entities, which may both present as ring-enhancing lesions on neuroradiology investigations. Understanding their natural history and diagnost… Show more

“…Low signal intensity was noted in the above areas on T1,T2 and T2 ∗ -weighted images [28]. The use of MRI is specifically important in TB of the central nervous system especially in neonates where it can present as ring-enhancing space-occupying lesions which are commonly shared with other bacterial infections or intracerebral hematomas [29].…”

“…Although we present a case of a 78-year-old patient, aspects of prognosis and morbidity may differ between populations of patients [5, 20]. The literature suggests a tendency to affect “frail subjects, with peaks in the elderly and pediatric populations [5, 24, 25]”; hence, a high index of suspicion should be considered for those at high risk. A systematic review of the literature on patients undergoing surgery for degenerative cervical myelopathy suggests that though duration and severity of symptoms related to neurological procedures are more closely associated with surgical outcomes, elderly patients and positive smoking status may have predictive value for comorbidities following surgery [7].…”

“…Neuroimaging of a brain abscess relies on the stage of the abscess at imaging and the etiology of the abscess. Brain abscess presents as clear ring-enhancing lesions (Figure 4B) (118,128,129) in DWI and perfusion imaging, which is clearly different from necrotic tumors.…”
