Right Ventricular Endomyocardial Fibrosis (2013 Grover Conference Series)

Abstract: Endomyocardial fibrosis (EMF) is a restrictive cardiomyopathy of unknown origin, affecting predominantly the right side of the heart. Its highest prevalence is in poor regions of sub-Saharan Africa, where it is usually found in children and adolescents. In areas where it is endemic, EMF is a major cause of heart failure and premature death. EMF of the right ventricle has unique clinical, electrocardiographic, and echocardiographic signs. Several features of advanced disease are not fully understood, the most s… Show more

“… 1 The right ventricle is the cardiac chamber most frequently affected in EMF, either in isolation or as part of bilateral disease. 2 Apical filling with fibrotic tissue of 1 or both ventricles is the hallmark of EMF, often associated with thrombus, calcification, and atrioventricular valve regurgitation, leading to typical symptoms of restrictive heart failure. 4 …”

“… 1 It is highly prevalent in regions close to the equator and impoverished areas of sub-Saharan Africa. 2 Additionally, its incidence rate is notably higher in Kerala, India. 3 EMF is characterized by apical filling with fibrotic tissue of 1 or both ventricles; it is often associated with thrombus and calcification.…”

“…The most frequently affected cardiac chamber is the RV, with better clinical tolerance 6 . Isolated or predominant left ventricular EMF shows earlier mortality due to severe PH 7 . The main causes of severe PH detected in left EMF are diastolic dysfunction and mitral valve abnormalities 8 .…”

“…Regarding the differential diagnosis, RV is not that dilated and LV wall thickness is not changed during 15 years, indicating that this case is neither ARVC nor HCM. Some case reports investigated and described the RV dominant restrictive pattern that caused the ascites [8]. Such RV dominant restrictive pattern is frequently observed in EMF, especially right EMF, an endemic disease in south India and Africa.…”
