Right coronary autoregulation in conscious, chronically instrumented dogs

Abstract: Right coronary (RC) autoregulation and right ventricular (RV) function were assessed in conscious dogs, chronically instrumented to measure RC flow and RC pressure (RCP) as a hydraulic occluder on the RC was inflated. Dogs were then anesthetized, and RC autoregulation and RV function were again assessed. In the conscious state, moderate RC autoregulation was present with closed loop gains (Gc) of 0.59–0.27 as RCP was reduced from 100 to 40 mmHg. In the anesthetized state, Gc was not significantly less than in … Show more

“…In RV pressure overload, the systolic perfusion gradient (Aortic pressure systole -RVSP)may be eliminated and the diastolic RCA perfusion pressure (Aortic pressure diastole -RV end diastolic pressure) be reduced, thereby impairing RCA flow 60, 61 . RV contractile function remains constant until RCA perfusion pressures fall below 50 mmHg 62 .…”

“…Right ventricular ischemia may reflect reduced right coronary artery perfusion pressure. However, right ventricular function is usually preserved as long as coronary perfusion pressure, the difference in pressure between the aorta and the right ventricle in systole or diastole, exceeds 50 mm Hg 221. In PAH, systemic (aortic) hypotension and a rise in right ventricular pressure (in both systole and diastole) combine to reduce the pressure gradient that drives perfusion of the right coronary artery, contributing to right ventricular ischemia.…”
