Right atrial function in hypertensive patients: effects of antihypertensive therapy

Abstract: Objectives: The effects (16 weeks) of oral antihypertensive drugs on right atrial (RA) function were evaluated by two-dimensional and Doppler echocardiography in 64 patients with mild-to-moderate essential hypertension. Thirty-two age-and sex-matched normal subjects served as controls. Background: Hypertension alters the diastolic properties of the left ventricle and disturbs the left atrial contractile activities. RA performance may also alter in essential hypertension. Methods: From the tricuspid flow veloci… Show more

“…Such structural remodeling leads to an increased incidence of AF in a dose-dependent response to cumulative hypertension burden, as shown in our study and by others [26]. As such, a change in left ventricular hypertrophy can be prevented or even improved with intensive BP control and anti-hypertensive medications [31,32], and strict BP control should lower the incidence of AF in patients with DM.…”

“…Although we and others [23] believe that the volumetric indexes we used provide a more direct and comprehensive assessment of right atrial function than Doppler measurements of tricuspid or hepatic vein flow, they are more dependent on acoustic windows and image quality than Doppler. We measured right atrial size and function in this study by retrospectively reviewing echocardiograms that were performed previously for clinical reasons.…”

“… 40 , 41 Right ventricular diastolic insufficiency, which usually appears earlier than right ventricular systolic insufficiency, right ventricular dilation or hypertrophy, may be clinically useful as an early and easily quantifiable subclinical marker of impaired right ventricular function. 42 , 43 This may be due to the insensitivity of the echocardiography parameters used in evaluating right heart diastolic function and limited value for estimating RV diastolicity because of the variation with loading conditions. Future studies with more sensitive evaluation parameters would be useful.…”
