Rifampicin-induced thrombocytopenia

Abstract: Summary Rifampicin occasionally causes thrombocytopenia when given as part of an intermittent regimen. A case is reported of severe thrombocytopenia developing after one dose of rifampicin, following a 4-month gap in daily therapy. The literature on rifampicin-induced thrombocytopenia is reviewed.

“…Hematologic monitoring may be done periodically in patients on long-term, intermittent therapy, particularly during initiation of therapy in patients who have previously received rifampin, as severe thrombocytopenia has been demonstrated in patients with gaps in therapy. 67 In patients with rifampin-associated thrombocytopenia on intermittent therapy, a switch to daily therapy may also be considered. 68…”

“…This finding correlates with previous studies reporting that intermittent drug administration is likely to result in a more severe and rapid adverse reaction. RFP-induced thrombocytopenia mostly developed in patients who were either on intermittent-dose regimens or were re-administered RFP after an interval (5,7,9,10).…”
