Rickettsia japonica infections in Huanggang, China, in 2021

Abstract: Two patients from Huanggang, China, were diagnosed with spotted fever group (SFG) rickettsiosis—caused by spotted fever group rickettsiae ( SFGR )—in 2021. This study aimed to investigate the clinical symptoms, laboratory examinations, epidemiological factors, and therapeutic responses in patients with SFG rickettsiosis—an emerging disease in this region. “…Rickettsia japonica has only been discovered once in north-eastern Europe [ 20 ], but the precise location, animal and tick source of the microorganism's DNA were not specified in this report. Rickettsia japonica is the causative agent of Japanese spotted fever (JSF), which is endemic in Japan [ 21 ]; in Europe, neither its vector, Haemaphysalis longicornis, nor a human case of JSF have ever been described.…”

“…It was reported that the seroprevalence rate of anti-Rickettsia japonica antibody is about 21% among people in Yichang, a city in the Yiling Mountains region of central China (12). The main clinical symptoms of tick-borne rickettsioses also include fever and thrombocytopenia as well as headache, muscle pain, rash and local lymphadenopathy (13)(14)(15)(16) (12,17). It is worth noting that central China including the Dabie and Yiling Mountains region is also known to be an important endemic area for orthohantavirus hantanense (HTNV) infections transmitted by rodents, which belongs to the order Bunyavirales, family Hantaviridae, and genus Orthohantavirus (18).…”

“…Spotted fever is caused by the spotted fever group Rickettsia species, which are mainly vectored and transmitted by ticks. In China, Japanese spotted fever (JSF) caused by Rickettsia japonica is a common disease in eastern and central areas such as Zhejiang, Hubei, and Henan provinces ( Lu et al., 2018 ; Li et al., 2019 ; Li and Liu, 2021 ; Gao et al., 2022 ; Zhou et al., 2023 ). The main manifestations of JSF are fever, erythema, and eschar ( Noguchi et al., 2018 ).…”
