Richter's Hernia in the Laparoscopic Era

Abstract: Richter's hernia can occur at trocar sites after laparoscopic procedures, and 10-mm or larger ports are the usual culprits. Most surgeons now routinely close the fascia of these sites to prevent herniation. The usual presentation is of crampy abdominal pain with nausea and vomiting. Treatment is reduction of the bowel that is incarcerated and then repair of the fascial defect. We describe four cases of Richter's hernia after laparoscopy, two that were repaired by open procedure and two that were repaired lapar… Show more

“…Richter’s hernia occurs in small hernia rings large enough to entrap the partial circumference of the bowel wall, but small enough to prevent protrusion of a loop of the intestine, with firm margins commonly occuring in the femoral ring (72 %-88 %), followed by inguinal canal (12-24 %) and the abdominal wall incisional hernias (4 %-25 %). Recently several cases have been reported at laparoscopic port insertion sites also [ 3 ]. Unusual occurrences occurs at the insertion site of the drainage tube following open abdominal surgery, as a Spigelian’s hernia, through the sacral foramen [ 1 ].…”

“…Importantly, incisional hernias can be associated with significant morbidity in the presence of small-bowel obstruction that, in addition to repair of the hernia, requires bowel resection. 13 Our objectives were to describe the frequency of hernia repairs after RP and estimate the impact of surgical approach on the likelihood of incisional hernia. We hypothesized that there would be a higher risk of incisional hernia repairs associated with MIRP compared to ORP.…”
