Rhinosinusitis: Developing guidance for clinical trials

Abstract: The Rhinosinusitis Initiative was developed by 5 national societies. The current guidance document is an expansion of the 2004 publication, "Rhinosinusitis: Establishing definitions for clinical research and patient care" and provides templates for clinical trials in antimicrobial, anti-inflammatory, and symptom-relieving therapies for the following: (1) acute presumed bacterial rhinosinusitis, (2) chronic rhinosinusitis (CRS) without nasal polyps, (3) CRS with nasal polyps, and (4) classic allergic fungal rhi… Show more

“…In the absence of specific intervention studies in COPD, nasal symptoms should currently be managed by reference to appropriate guidelines. 13 For those considering intervention studies, guidelines exist for the conduct of such research, 36 and it must be hoped that such studies are indeed performed given the urgent need for truly novel treatment strategies to treat this prevalent and devastating disease.…”

“…Guidelines for the diagnosis and treatment of rhinosinusitis differ in terms of diagnosis of CRS and therapy; oral and intranasal corticosteroids are the standard therapy for CRS and NP [6][7][8][9][10]. Glycosaminoglycans (GAGs) also known as mucopolysaccharides, are long unbranched chains of disaccharide units that are repeated in the determined order by alternating an amminosaccaride (containing an amino group instead of a simple-OH group) to a monosaccharide usually acid (containing one or more carboxyl groups and/or sulphates and thus negative charges).…”

“…CRS is one of the most common comorbidities among patients with asthma, and the comorbidities share similar inflammatory processes and histopathology (17). CRS has been recently divided into two subgroups: with and without nasal polyps (18). The majority of cases of CRS with nasal polyps show Th2-type inflammation with prominent accumulation of eosinophils in the nasal polyps and are thus called eosinophilic CRS (ECRS) (19).…”
