Rhinoplasty Complication Requiring Multiple Revisions

Abstract: Cosmetic rhinoplasties are complex surgical procedures that can present with serious complications requiring revision surgeries. Septal graft difficulties are common complications in rhinoplasties, requiring surgeon awareness of the potential future need for corrective surgery. Here, we report a case of multiple revision cosmetic rhinoplasties due to recurrent complications. Additionally, a review of the literature regarding cosmetic rhinoplasty complications and risk factors was performed.

“…Postoperative infections following rhinoplasty can lead to localized tissue deficiencies and, in severe instances, extend their pathological reach to adjacent anatomical structures, a process facilitated by the dense network of blood vessels and lymphatics in the facial region 13,14 . This propagation can affect structures including maxillary sinuses and even facial bones 1,2 . Such spread of infection can occur through contiguous tissues, exacerbating the severity of the condition and necessitating prompt and comprehensive management to prevent further complications.…”

Such extensions can provoke a cascade of inflammatory responses, leading to compounded tissue loss and presenting a clinical challenge that necessitates immediate and effective intervention.…”

“…Despite the development and evolution of several surgical techniques in septorhinoplasty, recurrent deviations still occur, with the rate of post-operative nasal deviation ranging from 0-11% 4,5 and pooled incidence of revision surgery around 0-10.9% 6 . The role of septorhinoplasty for adequate correction of deviated nose is well documented but the rationale and patterns for recurrences after surgery remain unclear 3 .…”

“…2 On the contrary, deviations of the lower two-thirds (including the septum) can be addressed with various techniques such as camouflaging cartilages, onlay grafts, spreader grafts, septal extension grafts, and septal relocation. 2 Despite the development and evolution of several surgical techniques in septorhinoplasty, recurrent deviations still occur, with the rate of postoperative nasal deviation ranging from 0 to 11% 4,5 and pooled incidence of revision surgery around 0 to 10.9%. 6 The role of septorhinoplasty for adequate correction of deviated nose is well documented, but the rationale and patterns for recurrences after surgery remain unclear.…”
