Rhabdomyosarcoma of the Adult Nasopharynx

Abstract: Rhabdomyosarcoma of the adult head and neck is rare, particularly beyond 40 years of age. In this region, the nasopharynx is an unusual site. Most nasopharyngeal rhabdomyosarcomas are of an alveolar variety. We report a case of embryonal or spindle cell rhabdomyosarcoma in the nasopharynx of a 47-year-old man. The histology of this tumour revealed clear cells that have not been described in embryonal rhabdomyosarcomas occurring in the head and neck. The pathology is discussed and current literature reviewed.

“…18 Alveolar rhabdomyosarcoma is also the predominant RMS type in the nasopharynx. 14 Our results, however, contrast with those of others who identified higher percentage of ERMS and mixed alveolar and embryonal patterns. In a review of 37 adults and children with sinonasal RMS, 16 cases were embryonal, 15 alveolar, 1 mixed, and 5 were unclassified.…”

“…The clearing of the cytoplasm is most likely due to the presence of glycogen and/or lipid 5 and has been observed in previous reports of sinonasal RMS. [14][15][16][17] The clearing of the cytoplasm, even though it might not bear any known clinical significance, can add to the diagnostic confusion. Clear cells were also seen in RMS of other head and neck areas.…”

“…17,18 This is not surprising since this is a rare tumor in the nasopharynx especially in the adult. 7 The cancers of the nasopharynx present with varying clinical features with majority being vague and nonspecific in early stage. The commonest clinical symptom in the present study was neck swelling which represents lymphadenopathy, a similar finding by several other researchers.…”

“…5,6 It is rare especially after 40years and the commonly found type in the adult nasopharynx is the alveolar variant. 7 Other malignancies such as adenocarcinomas often from salivary glands are rarely seen as primary malignant tumor of the nasopharynx. Nasopharyngeal carcinoma (NPC) by far is the commonest primary malignancy seen in the nasopharynx.…”
