Aims The assessment of functional status is a more appropriate measure in the older people than traditional healthcare outcomes. The present study aimed to analyse the association between functional status assessed using the Barthel Index and length of stay, in‐hospital mortality, discharge destination, and Diagnosis‐Related Groups‐based cost. Design This study was a retrospective study that used administrative data from patients older than 65 discharged from the University Hospital of Padua (Italy) in 2016. Methods A logistic regression model for categorical variables (length of stay, in‐hospital mortality, and discharge destination) and a generalized linear model with gamma distributions and log links for continuous variables (cost of hospitalization) were used to evaluate associations with the Barthel Index. Results A total of 13,484 admissions were included in the analysis. In‐hospital mortality, safe discharge, and length of stay were higher in patients with severe dependence than in patients with mild/no dependence with a 12‐fold increased risk of death (OR = 12.81; 95% CI 9.22–18.14), a 4 times greater likelihood of safe discharge (OR = 4.64; 95% CI 3.96–5.45), and a 2‐fold increase in length of stay (OR = 2.56; 95% CI 2.34–2.81). On the other hand, no significant association was found between the cost of hospitalization and the Barthel Index. Conclusions Barthel Index was strongly associated with in‐hospital mortality, discharge destination, and length of stay. The costs of hospitalization, however, were not related to patients’ functional impairment. Impact The study considers functional status as an indicator of hospital outcomes. Better comprehension of the relationship between functional status and healthcare outcomes may help with early and adequate healthcare planning and resource management.

As a study by Ozyemisci-Taskiran et al demonstrated that infections (in addition to admission motor impairment) affected length of stay in stroke rehabilitation patients (Ozyemisci-Taskiran, Gunendi, Aknar, Karatas, & Sepici, 2011), the importance of inflammation on this outcome measure is more clearly established. In the study described here, as neither improvement in a nutritional measure nor adequate nutritional intake universally corresponded to decreases in complications or length of stay while worsening of an inflammatory marker did correspond to increases in complications and length of stay, we must disagree with the studies linking malnutrition to poor performance in these outcome measures in the stroke patient.…”

BACKGROUND: Nutrition's impact on stroke rehabilitation outcomes is controversial. Existing studies utilize albumin without correcting for inflammation in nutritional assessments. Here, prealbumin was used and inflammation assessed to determine if nutrition impacts rehabilitation outcomes. OBJECTIVE: Determine the effect of dietary intake on prealbumin level, number of complications, length of stay, and Functional Independence Measure (FIM) efficiency in rehabilitation stroke inpatients. METHODS: Patients had admission and discharge prealbumin and C-reactive protein (CRP) levels drawn; and, weekly protein and calorie counts obtained. Patients were followed for number of complications, length of stay, and FIM efficiency. RESULTS: Mean protein and calorie intake was 57.6 ± 16.2 g/d and 1452.2 ± 435.8 kcal/d, respectively. 77.6% of patients had normal prealbumin on admission with 94.9% on discharge. Prealbumin increased significantly from admission to discharge (22.3 ± 6.2 mg/dL vs. 24.6 mg/dL ± 5.1 mg/dL, P = 0.007). Number of complications and length of stay were predicted by CRP in regression models. Total, motor, and cognitive FIM efficiencies were not universally affected by prealbumin levels, protein intake, or calorie intake. CONCLUSIONS: Nearly all hypoprealbuminemic stroke rehabilitation inpatients correct their levels eating a non-supplemented diet. Number of complications, length of stay, and functional outcomes in this patient are not affected by prealbumin levels, protein intake, or calorie intake.

ResumoIntrodução: O processo do envelhecimento associado às doenças crônico-degenerativas conduz ao declínio funcional e/ ou cognitivo, resultando em redução da qualidade de vida e diminuição da autonomia e independência. Abstract Introduction:The aging process associated with chronic degenerative diseases leads to functional decline and/or cognitive impairment resulting in reduced quality of life and decreased autonomy and independence. Objective: To compare the functional and cognitive capacity of elderly in a geriatric rehabilitation unit in the city of Salvador, Bahia. Methodology: This is an observational descriptive cross-sectional analysis using medical records. The medical records of elderly patients admitted in the Rehabilitation Unit of the Geriatric Center of Obras Sociais Irmã Dulce were selected from January to December 2011. The charts were evaluated for sociodemographic, length of stay, clinical diagnosis, scores of the Modified Barthel Index (MBI) and the Mini-Mental State Examination (MMSE) on admission and discharge. Results: A total of 38 records met the inclusion criteria. 73.68% of the patients were male. The age ranged from 62 to 94 years and length of stay from 34 to 179 days. The most prevalent diseases were hypertension (68.42%), stroke (57.89%), restriction of mobility (42.10%), double incontinence (28.94%), diabetes mellitus (23.98 %) and vascular dementia (21.05%). The observed changes in the MBI scores did not change the functional levels of the elderly. Cognitive changes were observed in 81.57% on admission and 63.15% on discharge. Only individuals with 1-3 years of education changed levels of cognition. Conclusion: The process of aging associated with chronic degenerative diseases influences on the functionality and cognition of the studied population. The results provided evidence for the establishment of goals and treatment plans that promote the rehabilitation of the elderly.
