Revisional paraesophageal hernia repair outcomes compare favorably to initial operations

Abstract: Recurrent PEH repair is performed with similar outcomes to IPEH repair with the exception of increased operative time and blood loss. Collis gastroplasty is required more frequently in RPEH patients. Persistent symptoms exist following RPEH repair. Despite recurrent symptomatology, patient satisfaction is high. RPEH repair may be safely performed in patients with recurrent paraesophageal hernias with outcomes similar to IPEH repairs.

“…This evaluation of patients requiring redo PEHR demonstrate favorable outcomes after laparoscopic redo fundoplication with similar complication rates and resolution of symptoms as patients with initial PEHR. The findings confirm those from recent published series from Zahiri et al 4 and Wennergen et al 5 that provided comparable short-term outcomes, patient satisfaction, and quality of life between initial and redo PEHR after one-year follow-up. Long-term follow-up after mean 48.3 months revealed similar rates of hernia recurrence on objective imaging and reoperation.…”

“…3 Recent studies have demonstrated comparable rates of short-term symptom resolution and quality of life after redo PEHR compared with initial PEHR. 4,5 Thus, long-term rates of symptom resolution and hernia recurrence after redo PEHR, as compared with initial PEHR, remain poorly understood.…”

“…Regarding the preferred surgical technique for redo surgery, in our series Nissen fundoplication was the first option (42.9%), whereas the second choice was Toupet (30.4%). No case of Collis gastroplasty was performed, in contrast to Wennergren et al, 18 who reported that Collis gastroplasty was the more frequent technique in patients undergoing redo. However, we have noticed an increase in the number of Toupet fundoplication (61.1%) in the last five years.…”

“…A review of initial versus reoperative laparoscopic hiatal hernia repairs by Wennergren et al found an increase in radiographic recurrence as well as persistent symptoms in patients undergoing repeat repair. 15 The fact that more recurrent repairs were performed robotically highlights several strengths of the robotic technique. First, surgeons are more confident in pursuing a minimally invasive approach in these patients knowing the capabilities of the robot.…”
