Revisional Bariatric Surgery for Unsuccessful Weight Loss and Complications

Abstract: Revisional bariatric surgery effectively treated the undesirable results from primary bariatric surgery. Laparoscopic revisional surgery can be performed after both failed open and laparoscopic bariatric procedures without a prohibitive complication rate. Carefully selected patients undergoing revision for weight regain have satisfactory additional weight loss.

“…Obesity surgery literature shows that although the procedure is the best evidence-based obesity treatment for weight loss and weight loss maintenance to date, 20-30% of people start to regain weight within 24 months [29]. Numbers of patients requiring revision surgery due to weight regain following their first procedures are growing with incident rates ranging from 5 to 56% [30,31]. Gastric bands patients report higher revision surgery rates [32].…”

“…In our experience, RYGB is the procedure of choice for revising previous bari atric surgery, specifically VBG, because of its effective weight loss results and safety. [23][24][25] Not surprisingly, patients who underwent VBG most often qualified for further surgery (23% underwent RYGB), whereas most patients who had anatomically successful operations, such as prior SG or RYGB, were unlikely to need further surgical management (92%). In these patients, we often repaired hernias and performed panniculectomies.…”

“…Marceau et al reported excellent 20 years results after BPD-DS with a %EWL of over 70% [3], but Biron et al mentioned that weight regains after BPD-DS is an increasing problem, especially in super-obese patients, doubling the failure rate every five years with weight regain [16]. In our series, we identified 17 patients with weight regain at a median follow-up of more than 5 years In bariatric surgery, weight regain is a common problem [7,17,18]. Even after satisfactory weight loss, weight regain can occur if patients go back to bad eating habits and sedentary lifestyle [8,19].…”
