Review Paper. Transplacental Digoxin Treatment In Prenatal Cardiac Problems In Singleton Pregnancies - Meta Analysis (Based On Literature: 1992–2015)

Abstract: Based on fourteen case reports from various centres from 1992-2015 and three original studies in [2006][2007][2008][2009][2010][2011] 122 fetuses were subjected to analysis. In these reports, transplacental digoxin treatment was administered to different cardiac anomalies such as SVT , Ebstein's anomaly, critical AS , absent pulmonary valve syndrome, complete heart block, in foetuses with aneurysm/diverticulum of LV, in tricuspid atresia or dysplasia, rhabdomyoma, pulmonary atresia, HLHS with fibroelastosis, … Show more

“…Although fetal cardiac symptoms are usually bad indicators for prognosis, we report for the first time the value of transplacental digoxin treatment in these cases as early as possible and for as long as possible. Digoxin is usually well accepted by pregnant woman; it was proven in several other fetal cardiac conditions [14]. In this series of cases with AVGM an improvement in CVPS was seen in two survivors and no improvement was observed in non-survivors (Table 2).…”

“…FHR measurement is important for preparing an accurate prenatal medical ultrasound report. Although our fetal cardiology referral center deals with various types of fetal arrhythmia and their pharmacological treatment, FHR measurement was absent from more than 50% of cases present in the database [23][24][25]. Our findings demonstrate the need to improve this approach; for example, patients with FHR just below the bottom 5th percentile should be candidates for further cardiological work-up, e.g electrocardiogram with the QTc evaluation.…”

“…Our patients received postoperatively relatively high doses of digoxin, which could raise safety-related concern. The chosen dosing was based on the literature review [ 27 , 44 , 45 ] and it has its background on given in the literature rate of the drug passing through the placenta to the fetus (feto-maternal ratio). Our results in this field will be addressed in another paper [ 46 ].…”
