Abstract. This is the third part of the review-tutorial paper describing fundamentals of tissue optics and photonics. The first part of the paper was mostly devoted to description of tissue structures and their specificity related to interactions with light [1]. The second part presented light-tissue interactions originated from tissue dispersion, scattering, and absorption properties, including light reflection and refraction, absorption, elastic, and quasi-elastic scattering [2]. This last part of the paper, underlines mostly photothermal and nonlinear interactions such as temperature rise and tissue damage, photoacoustic and acoustooptical, nonlinear sonoluminescence, Raman scattering, multiphoton autofluorescence, second harmonic generation (SHG), terahertz (THz) radiation interactions, and finally photochemical interactions with description of two widely spread therapeutic applications: photodynamic therapy (PDT) and low level light therapy (LLLT). © 2016 Journal of Biomedical Photonics & Engineering.Keywords: tissue optics, photothermal effects, nonlinear interactions, temperature, tissue damage, photoacoustics, acoustooptics, sonoluminescence, Raman scattering, multiphoton autofluorescence, SHG, terahertz, photochemical processes, PDT, LLLT. 1(1), 3-21 (2015). 2. Introduction: Anastomotic leak (AL) after surgery for esophageal cancer remains a main cause of postoperative morbidity and mortality. Poor tissue perfusion at the site of anastomosis is one of the major factors for leak. We aimed to review the results of Indocyanine Green dye (ICG) with a goal to decrease the leak rate. Methods: Patients with cancer of esophagus and gastroesophageal junction were subjected to either upfront surgery or preoperative chemoradiation/ chemotherapy followed by surgery. Stomach was used for reconstruction. Intravenous injection of 5 mg – 10 mg ICG was given and vascular perfusion was assessed with infrared light of laparoscopic telescope. Gastroesophageal anastomosis was made at the site of adequate ICG perfusion. These patients (ICG group) was compared to the other group of patients in whom ICG was not used (Non-ICG group). Results: 28 and 396 patients belonged to ICG and Non-ICG group, respectively. 61% in ICG group and 32% in Non-ICG group had preoperative treatment (p <.001). AL was observed in 7% and 16% in ICG and Non-ICG group, respectively (p = 0.2). Healing time of leak was 15 days in ICG group and 32 days in Non-ICG group (p = .03). One patient required revision of anastomotic site based on ICG finding. There was no adverse reaction related to ICG injection. Conclusion: Fluoroscence angiography using ICG is a safe method for evaluation of vascular perfusion of gastric conduit. Though the leak rate was not statistically different in the two groups, ICG group required lesser time for complete resolution of AL, which might indicate lesser severity of anastomotic disruption.

The results of evaluation of the soft tissue perfusion of the foot in patients with diabetes and critical lower limb ischemia (CLI) via fluorescence angiography (FAG) in the near-infrared range are represented in the article. The study included 4 diabetic patients with CLI. All patients underwent lower-limb balloon percutaneous transluminal angioplasty. To evaluate the soft tissue perfusion of the foot via FAG the following parameters have been used: T 0m -time to reach maximum for fluorescence intensity after intravenous administration of Indocyanine green; T Im -time of onset of maximum fluorescence intensity after its appearance in the area of interest; I m -the level of the maximum fluorescence intensity. The region of interest was the skin around the wound of the foot. The median FAG parameters in the region of interest on the foot were as follows: prior to surgery T 0m = 164 sec (148-181), T Im = 48 sec (38-56); after surgery: T 0m = 80 sec (69-92); T Im = 27 sec (20-39); (p <0,05). No adverse reactions were registered in the study. Despite the small sample we were able to achieve statistical significance in the difference between the parameters prior to and after surgery. Further prospective studies with extended samples of patients are needed. В исследование были включены 4 пациента с сахарным диабетом и критической ишемией нижних конечностей. Всем пациентам была выполнена чрескожная транслюминальная баллонная ангиопластика артерий нижних конечностей. Для оценки перфузии тканей стопы при помощи метода флуоресцентной ангиографии были определены следующие параметры: T 0m -время достижения максимальной интенсивности флуоресценции с момента внутривенного введения индоцианина зеленого; T Im -время наступления максимальной интенсивности флуоресценции с момента ее появления в зоне интереса; I m -уровень максимальной интенсивности флуоресценции. Зоной интереса являлась кожа в околораневой зоне стопы.
