Reversing dobutamine‐induced tachycardia using ivabradine increases stroke volume with neutral effect on cardiac energetics in left ventricular post‐ischaemia dysfunction
Abstract:A combined infusion of dobutamine and ivabradine had a neutral effect on post-ischaemia LV efficiency and increased left ventricular output without an increase in HR.
“…In one experimental swine model, Bakkehaug et al [25] presented an ischemia-reperfusion protocol in 12 pigs after intermittent ligation of the left coronary arteries. Dobutamine infusion in post-ischemic heart increased CO by increasing significantly HR from 102 to 131 bpm (p < 0.05).…”

Ivabradine reduces heart rate and increases stroke volume without modifying systemic or left filling pressures in a swine model of acute heart failure. However, an excessive heart rate reduction could lead to a decrease in cardiac output and an increase in right filling pressures. Future studies with specific heart rate targets are needed.

“…In conclusion, results presented in the paper of Bakkehaug et al . () are promising and have to be confirmed by a clinical study to assess the effect of this strategy in the aim to counteract deleterious effects of dobutamine in patients with CS all the more as we lack data and because of poor outcomes in this setting.…”
