Reversible splenial lesion in new-onset refractory status epilepticus: A case report

Abstract: NORSE is a rare condition, which causes refractory seizures in healthy individuals. Abnormalities in the corpus callosum on MRI have not been reported in NORSE patients. We report a case of NORSE presenting with a reversible splenial lesion on brain MRI. A reversible splenial lesion may result in NORSE.

“…10,56,57 Unspecified autoimmune etiology was classified as such when investigation panels rendered seronegative results, but where an autoimmune basis was strongly suspected: a diagnosis based on the "holistic" view of the clinical presentation, taking into consideration the history, imaging result, cerebrospinal fluid (CSF) analysis, and exclusion of other etiologies. 58,59 Genetic causes were mitochondrial in origin, 60,61 vascular comprised ischemic pathologies, 23,26,62 structural were any neuroanatomical issues, such as reversible corpus callosum lesions, 63,64 and toxins included synthetic cannabinoid use and alcohol. 6,10,65 All included etiologies were identified only after a negative initial diagnostic workup in the first 72 h of presentation.…”

“…Second, the finding of a reversible splenial lesion was quite unusual among cases of FIRES, which is a very rare disorder per se. Aside from a 30-year-old patient with new-onset refractory status epilepticus [73], only two FIRES cases with this complication, including one without descriptions of the blood examination results [12], were identified (Supplementary Table S2). Consequently, we were unable to characterize this FIRES subgroup by comparing clinical and laboratory data between patients with and those without a splenial lesion.…”
