Reverse shoulder arthroplasty after failed megaprosthesis for osteosarcoma of the proximal humerus: A case report and review of literature

Abstract: The involvement of proximal humerus by Osteosarcoma is quite common, with the survival rates highly dependent on the staging of the tumor. Numerous salvage methods have been described but without any consensus. We present a case of a 17-year-old patient who had a wide excision of the tumor and underwent a mega prosthesis fixation, which underwent implant failure with the revised prosthesis subluxing. Finally, Reverse Shoulder Arthroplasty was done. Ten years follow up shows the success of the above. Salvage su… Show more

“…The patient has undergone tumor resection, partial shoulder replacement megaprosthesis since this solution can ensure wide resection, but due to the loss of function of the rotator cuff tendon, which causes dislocation of the proximal humerus upward leading to pain and severe arm movement limitation. The patient was replaced with an inverted shoulder joint in 2017, which improved the function of the shoulder joint [ 29 ].…”

“…There are currently some approaches available for the repair of tumorous defects involving the metaphysis, such as autograft, allograft, and prostheses ( Ruggieri et al, 2011 ; Wieser et al, 2013 ; King et al, 2016 ; Maclean et al, 2017 ; Rafalla and Abdullah, 2017 ; Nota et al, 2018 ; Chauhan et al, 2019 ). Autologous fibula graft has been widely applied in clinical settings because of its excellent biocompatibility and osteoinductivity ( Li et al, 2012 ; Pilge et al, 2018 ).…”

“…Although there is no consensus about the ideal method for reconstruction, proximal humeral replacement (PHR) with hemiarthroplasty is one of the most popular methods because of its convenience, good cosmetic appearance, and acceptable functional outcomes of the elbow and hand [1,3]. However, the function of the shoulder varies greatly among patients depending on the function of the axillary nerve and muscle reconstruction [6,[14][15][16]. Glenohumeral arthrodesis with bone grafts can achieve permanent stability once the graft heals and might improve the motion of the arm via the movement of the scapula [12,13,17].…”
