Reverse Foam Sclerotherapy of the Great Saphenous Vein with Sapheno-Femoral Ligation Compared to Standard and Invagination Stripping: a Prospective Clinical Series

Abstract: Standard stripping of the GSV and invagination stripping are not associated with major discomfort and problems in the early post-operative period. SFJ ligation and GSV reverse foam sclerotherapy yielded greater patient satisfaction with less post-op bruising and discomfort and reduced analgesic requirements.

“…The M:F ratio found in our study is in sharp contrast to the ratio mentioned in most of the western literature, where more females were suffering from this disease. 8,11,12,18,21,26 However in the study conducted by Jain et al, 41 of the 42 patients were males; by Masuda et al 42 of the 68 patients were males. 12,13 Majority of our patients were young with a mean age of 35.32 years in both group.…”

“…This is similar to many studies which have quoted a significantly less time in doing UGFS compared to surgery. 11,18,21,22 Complications Some complications were exclusive to surgery group and not seen on UGFS group. These included stitch infection in 3 (12%) patients, seroma in 3 (12%) and hematoma in 2 (8%) patients at 1 week follow up.…”

“…26 In the second study, Franek et al 26 compared the effects of treatment of 110 patients with VLUs divided into four comparable groups. Fifty-five patients divided into two groups had undergone VS with the modified Babcock's [29][30][31] method. One group (27 patients) received only SWC (the VS group) and the other group (28 patients) received SWC plus HVPC (the VS + HVPC group).…”

“…Further support on the short-term benefit of foam sclerotherapy comes from another study that randomized patients with varicose veins along the distribution of the GSV in three groups of 30 patients each. 10 The three procedures in the study consisted of flush saphenofemoral ligation with division of tributaries followed by either: (a) standard stripping of the GSV; (b) invagination stripping of the GSV using a pin stripper; or (c) reverse foam obliteration of the GSV via an angiography catheter that cannulated distally the divided saphenofemoral junction and was advanced approximately 10 cm above the knee. The foam group at two weeks follow-up was associated with significantly less blood loss, bruising and discomfort, and reduced analgesic requirements than either of the stripping techniques.…”
