Reversal of warfarin anticoagulation for urgent surgical procedures

Abstract: Purpose Patients treated with warfarin for therapeutic anticoagulation present a challenge for the perioperative management of urgent and emergent surgery. Anticoagulation must be reversed prior to most surgical procedures to prevent intraoperative bleeding. The purpose of this module is to review the options for urgent reversal of warfarin anticoagulation and the indications for each reversal agent. Selection of the appropriate agent is important to reduce unnecessary complications of treatment and to achieve… Show more

“…Given that anticoagulation reversal is thought to lower the risk for blood loss and transfusions, it was unexpected that there were no significant differences in blood loss or transfusions between groups. 16,19,29 This may be due to the mean pre-operative hemoglobin being significantly lower for the reversed group than the not reversed group, p < 0.001. The unadjusted volume of pRBC transfusions approached statistical significance and was higher for those reversed than those not reversed, p ¼ 0.03.…”

“…[ 10 12 ] It is also used to reverse the increase in INR caused by warfarin during the perioperative period of surgery, so as to reduce the risk of bleeding in emergency surgery. [ 13 ] Invasive emergency surgery should be performed at 6 to 12 hours after intravenous administration of Vitk1 10 mg. However, for patients receiving warfarin with an INR at the treatment window (2.0–3.0) and below, the proportion of spontaneous bleeding is very low.…”

“…Urgent surgical intervention of acute SSDH may be complicated by patient use of anticoagulants, as demonstrated above. Emergency reversal of warfarin9 and heparins10 have been well documented, however there were no definitive reversal agents of NOACs until andexanet alfa was trialled in 2015 11. A comprehensive review12 published this year explored the reversal of all classes of anticoagulants and highlighted the encouraging results seen by andexanet alfa in trials such as ANNEXA-4 13.…”
