Reversal of Type 2 Diabetes Mellitus and Improvements in Cardiovascular Risk Factors After Surgical Weight Loss in Adolescents

Abstract: Extremely obese diabetic adolescents experience significant weight loss and remission of type 2 diabetes mellitus after Roux-en-Y gastric bypass. Improvements in insulin resistance, beta-cell function, and cardiovascular risk factors support Roux-en-Y gastric bypass as an intervention that improves the health of these adolescents. Although the long-term efficacy of Roux-en-Y gastric bypass is not known, these findings suggest that Roux-en-Y gastric bypass is an effective option for the treatment of extremely o… Show more

“…However, the optimal age in adolescence for bariatric surgery is as yet undetermined [24] . Our findings, and those of others [26,27] indicate that bariatric surgery before adulthood can substantially reduce weight [10,25] and resolve comorbidities [25] . Moreover, earlier treatment of obesity may prevent later costs.…”

“…Bariatric surgery may be a feasible alternative, with one study demonstrating reversal of type 2 diabetes in ten of 11 adolescents [37]. However, our study demonstrates that, if adhered to, a VLED may be as effective and possibly safer for youth, in whom the long-term effects of bariatric surgery, both physical and mental, are largely unknown [38].…”

“…In a meta-analysis done by Black and colleagues, there was a significant change in the BMI at one year following bariatric surgery with the largest reduction in BMI noticed with RYGB (72). There is some evidence to suggest that bariatric surgery can lead to resolution of associated co-morbidities such as sleep apnea, type 2 diabetes and other cardiovascular risk factors (77,78). Bariatric surgery has also been reported to improve insulin sensitivity and secretion (79).…”
