Reversal of Atrial Remodeling after Cardioversion of Persistent Atrial Fibrillation Measured with Magnetocardiography

Abstract: Magnetocardiographically detected atrial electrophysiologic alterations in persistent AF diminish rapidly although incompletely during maintained SR after CV. This might be related to the known early high and late lower, but still existent tendency to AF relapses.

“…26 The association of the ACE DD genotype with a higher rate of recurring AF found in this study supports this concept. Similarly, a very recent study demonstrated that higher transforming growth factor-β1 levels are associated with AF recurrence after catheter ablation.…”

“…Indeed, although experience is still limited to only few institution, contactless MCG is one of the most promising technology for non‐invasive cardiac electro‐anatomical imaging and accurate localization of arrhythmogenic substrates (Brisinda, Venuti, Sorbo, & Fenici, ; Fenici & Brisinda, ; Fenici, Brisinda, Venuti, & Sorbo, ; Kwong, Leithäuser, Park, & Yu, ). Several studies, all carried out in magnetically shielded rooms, have demonstrated the usefulness of MCG to study AF (Jurkko et al, , ; Kim & Ahn, ; Kim, Kim, Lee, & Ahn, ; Koskinen et al, ; Lehto et al, ; Lim et al, ; Mäntynen et al, ; Nakai et al, ; Sato et al, ; Yamada & Yamaguchi, ; Yamada, Tsukada, Miyashita, Kuga, & Yamaguchi, ; Yoshida et al, ). The reproducibility of automatically measured terminal high‐frequency component of signal averaged atrial magnetic signals was evaluated in PAF patients compared with controls (Koskinen et al, ).…”

“…Among different analytic approaches used to evaluate the predictive value of MCG for AF occurrence and recurrence, pseudo‐current reconstruction was reported as sensitive method to non‐invasively differentiate inter‐atrial conduction patterns in normal subjects as well as in patients with PAF (Jurkko et al, ; Koskinen et al, ; Lehto et al, ; Mäntynen et al, ; Sato et al, ). Delayed atrial conduction along in the Bachmann’s bundle (BB), prevalence of inter‐atrial conduction at the fossa ovalis (FO), or multisite inter‐atrial conduction pattern were associated with PAF (Jurkko et al, ).…”

“…Thus altered and alternative conduction pathways may contribute to pathogenesis of lone AF [59]. Interestingly, in patients with persistent AF after cardioversion, MCG but not signal-averaged ECG or echocardiography identified electrophysiological alterations with incomplete recovery after 1 month in stable sinus rhythm [60].…”
