Reversal of acute renal failure by kidney revascularisation

Abstract: Patients with acute renal function deterioration due to ischemia of a single or both kidneys can benefit from prompt revascularisation, with significant recovery of renal function in most of them.

“…Treatment of RAS has shifted from surgical to endovascular procedures in recent years, with 95% of revascularized US Medicare patients undergoing angioplasty/stenting in 1999 [5]. These endovascular techniques have been used with success in acute RAO [2,7,8]. We have shown here that the management of anuric ARF in ARVD demands urgent intervention, but the value of renal revascularization in patients with more chronic presentations (e.g.…”

“…The limited literature suggests the more prompt the diagnosis and treatment initiated in anuric ARF, the better the renal outcome. Successful long-term preservation of renal function can be predicted by early intervention [8], absence of pre-existing renal impairment and by anatomic factors, in particular, when bilateral revascularization is necessary. While extreme renal dysfunction generally is predictive of poor long-term outcome in all patients with ARVD, a subset of patients will experience favourable results, even to the extent of rescue from dialysis as in this case.…”

“…13 Other studies have found that a significant proportion of patients requiring dialysis preoperatively were liberated following open revascularization (62.5-80%). 14,15 Although major trials have shown that renal artery stenting is not beneficial for RAS, 16,17 case reports demonstrate that angioplasty and stenting can successfully treat patients J o u r n a l P r e -p r o o f with RAO. [18][19][20] Catheter-directed thrombolysis is another alternative that has been shown to produce good angiographic results, but with decreased kidney function over time.…”

“…A second apparent paradox is that the occurrence of rapidly developing anuria can play a protective role, as demonstrated by some reports 54,55 showing a total success rate in recovering renal function in anuric patients, even after several months of dialysis treatment.…”
