Return to sports after ACL injury 5 years from now: 10 things we must do

Abstract: Background The outcome after ACL reconstruction (ACLR) is in general disappointing with unacceptable number of athletes that do not return to pre-injury level of sports, high re-injury rates, early development of osteoarthritis and shorter careers. Athletes after ACLR have high expectation to return to sports which is in contrast with the current outcomes. The aim of this manuscript is to present an overview of factors that are needed to be incorporated and to personalize the rehabilitation pro… Show more

“…From a practical point of view, football coaches might add the quantitative information of each player to personalize their screening activity towards ACL injury prevention. Despite ACL injury prevention programs which have sensibly improved over the years in young footballers (e.g., the FIFA 11+ [ 46 ]), such technologies might support dealing with the complexity of the football environment interactions [ 47 ]. For example, Cluster 1 players had the largest sample size in the present study cohort: the kinematics (and kinetics) of these players might be interpreted as a benchmark for the female footballers, with a protective motor strategy towards the ACL injury, and used as an example for more at-risk players [ 48 ].…”

“…Furthermore, most tests are not sports-specific in that they do not incorporate the demands present during sport, including the cognitive loads of attending to other players and tasks. Combining recent recommendations on optimizing RTS, future directions include integration of cognitive stimulation during motor tasks ( 65 , 66 ). Figure 2 introduces the combined elements of cognitive performance, movement quality, and motor performance that we suggest for functional RTS testing.…”
